Required Documents :

  1. Before coming for your medical examination, you must lodge your visa application to Canadian Embassy.
  2. Bring form Medical Examination from Canadian Embassy.
  3. Bring valid passport or copy, original national ID and birth certificate.
  4. Five pieces (5) recent passport – sized 4×6 (white background)
  5. Vaccination record if applicable
  6. Furtherance letter, if applicable

Whenever applicable, please bring the following :

  1. Old chest x – ray films not less than 6 months prior to the medical examination
  2. Detailed medical certificate regarding a previously – treated or currently managed clinically significant illness which should contain the following information :
  • Duration and date of treatment
  • Surgical procedure or medical intervention done
  • Complete final diagnosis
  • Outcome of treatment or treatment plans and prognosis, If available

*Chest X-ray may also be required for children 11 years old and below if they have had exposure to TB cases or history of TB disease

*Pregnant applicants will not conduct the Chest X-Ray unless under supervision and approval by Obstetric Gynecologist physician.

REMINDERS :

  • Female applicants within the reproductive age group are advise not to schedule their medical examination during their menstrual period. Otherwise, they will be required to come back one week after the last day of their menstruation.
